Instructions:
1.
This form is
required for any changes to team rosters after a season roster has been
submitted.
2.
This form MUST be
correctly completed and signed before
player(s) may participate as a member of the team he/she is joining. Otherwise
the player(s) may only participate as a substitute.
3.
The approved form
must be available for inspection by the opposing captain and will be mailed to
the scorekeeper along with the score sheet for the match.
4.
Annual Dues and
Season Fees, as applicable, for the added member(s) must accompany the Roster
Change Request and score sheet.
5.
Failure to comply
with proper Roster Change procedure may result in forfeiture of all games in
which the illegal player participated and revocation of any personal points
scored by the player.
6.
Players on a team
roster must be officially dropped before they can be added to the roster of
another team. Such a switch must be approved by the Board and requires a
minimum of eight (8) calendar days.
